Who Is Eligible?
You may be eligible to file a claim if you:
- You received a notice from On Q Financial (On Q Financial LLC) about the data security incident
- The breach, discovered on or around February 20, 2024, involved an unauthorized party accessing files that contained client names and Social Security numbers
- No documentation is needed to qualify for the flat alternative cash payment or the credit-monitoring benefit
- Documentation (statements, invoices, receipts) is required only if you claim reimbursement for documented out-of-pocket losses
How Much Can You Get?
On Q Financial agreed to a $1.25 million settlement fund. You can claim cash in one of two ways. The documented-loss payment reimburses up to $5,000 for actual, unreimbursed out-of-pocket costs fairly traceable to the breach — for example bank or telephone charges, the cost of credit reports or credit monitoring bought between February 20, 2024 and October 28, 2026, identity-theft expenses, and time spent dealing with the incident — but it requires supporting documentation. If you do not have documentation, or prefer a simpler claim, you can instead take an estimated $50 alternative cash payment, which may be adjusted on a pro-rata basis depending on how many people file. On top of either cash option, you can add expanded identity-theft protection: one year of single-bureau credit monitoring, up to $1 million in insurance for fraud or identity-theft losses, and access to a dedicated fraud-resolution specialist. No documentation is required for the monitoring benefit. Cash is paid by electronic transfer (if you file online) or by a mailed check. How to File Your Claim
File with the administrator:
- File a claim online, or download and mail the PDF claim form, at the official settlement website: onqsettlement.com
- Choose your benefit: up to $5,000 for documented out-of-pocket losses (with proof), or an estimated $50 flat alternative cash payment (no proof)
- You can add one year of single-bureau credit monitoring — which includes up to $1 million in identity-theft insurance and a dedicated fraud-resolution specialist — to either cash option, with no documentation required
- If you claim documented losses, attach supporting documents such as bank or credit card statements, invoices, telephone records, or receipts; if the documentation is found insufficient, the claim is treated as the $50 alternative cash payment
- Pick your payout method: electronic transfer (if filing online) or a paper check mailed to you
- Submit your claim by October 28, 2026; the deadline to opt out or object is earlier, on September 28, 2026, and the final approval hearing is November 16, 2026
