DaVita Ransomware Data Breach Settlement
By the Class Action Buddy Editorial Team · Last reviewed November 6, 2026
Most breach notices list names, addresses and Social Security numbers, and you can read a hundred of them without learning anything about the company. This one is different, and it is worth knowing exactly what went out of the door.
Alongside the usual identity data, the April 2025 ransomware attack on DaVita reached clinical information: health conditions, treatment details and dialysis lab test results. For some people it included tax identification numbers. In a limited number of cases it included images of checks written to DaVita, which means a routing number, an account number and a signature in one picture. DaVita is the largest dialysis provider in the country, so the people affected are largely patients with a chronic condition on record.
The benefits stack, which is unusual and easy to miss. You can claim the estimated $50 pro rata cash payment, the three years of credit monitoring and up to $2,500 of documented out-of-pocket losses on a single form rather than picking one.
Read the monitoring offer for what it is, though. It is one-bureau coverage, not the three-bureau kind other settlements provide.

Who Is Eligible?
You may be eligible to file a claim if you:
- Any person in the United States whose personal information was potentially accessed without authorization in the attack. The class is drawn that broadly.
- The attack was in April 2025, and was criminal ransomware against DaVita’s systems.
- What was exposed varied by person, and could include names, addresses, dates of birth, Social Security numbers, health insurance information and internal DaVita identifiers.
- It also reached clinical information — health conditions, other treatment information and dialysis lab test results. For some people, tax identification numbers. In limited cases, images of checks written to DaVita, which carry a routing number, an account number and a signature together.
How Much Can You Get?
A settlement not to exceed $15,000,000. Note that figure if you have read $10 million elsewhere; $15 million is what the administrator states.
You can claim any or all three benefits. They are not alternatives, which is easy to miss on the form:
A pro rata cash payment currently estimated at $50. “Estimated” is doing real work there — it falls if the total value of valid claims runs high.
Three years of credit monitoring and identity theft restoration. Read the detail: this is one-bureau coverage, not the three-bureau monitoring other breach settlements provide.
Up to $2,500 for documented out-of-pocket losses, also subject to pro rata reduction.

Frequently Asked Questions
What is the deadline to file a claim for the DaVita Ransomware Data Breach Settlement?
The current deadline to file a claim is Jan 19, 2027. Claims sent by mail must be postmarked by this date, so allow about a week for delivery if you are not filing online.
How do I file a claim for the DaVita Ransomware Data Breach Settlement?
File directly with the settlement administrator, using the official claim form linked on this page. Class Action Buddy does not support this settlement in the app, so the administrator’s own claim process is the route. You do not need a Claimant ID or PIN to file.
Do I have to pick between the $50 and the monitoring?
No. The settlement lets you submit a claim for any or all of the three benefits on a single form: the estimated $50 pro rata cash payment, the three years of monitoring, and up to $2,500 of documented out-of-pocket losses. People routinely select one and leave the rest.
Was my medical information really involved?
It varied by individual, but the categories the administrator lists include health conditions, treatment information and dialysis lab test results, which is clinical data rather than just identity data. For a smaller group it also included tax identification numbers and, in limited cases, images of checks written to DaVita.
Is the credit monitoring worth taking?
It is three years at no cost, so probably yes, but go in knowing it is one-bureau monitoring. Several other breach settlements this year offered three-bureau cover with identity theft insurance, and this is a thinner offer than those.
Why might the $50 shrink?
Because it is a pro rata payment against a capped fund. The administrator calls $50 an estimate, and both the cash payment and the documented-loss reimbursement may be reduced depending on the total value of timely valid claims received.
