How Much Can You Get?
Reimbursement of the $15 co-pays listed on your pre-populated Claim Form, under a $53,000,000 liability cap. With proof of payment (a receipt, cancelled check, bank or credit card statement, or provider letter), each documented co-pay is reimbursed in full. Without proof, a signed affirmation qualifies you for up to 40% of those co-pays, capped at $100 per class member. If approved claims exceed the net amount, payments reduce pro rata; money left over returns to the defendants.How to File Your Claim
You will need the Claim ID and PIN printed on the notice the administrator sent you. This settlement is not open to anyone who simply believes they qualify.
